Registration:
 
To apply for registration please book an appointment to see the practice administrator. She will need to see photo ID and current proof of address. The Health Care Assistant will conduct the new patient examination. Please bring a specimen of urine, a list of your repeat medications and childrens record of immunisation to your appointment.

 

Change Of Address:
 
If you move house and change telephone number please inform the surgery so that your computer records are kept updated. If you are not within our boundaries we may not be able to keep you on the register. You will be advised to change doctors.
 

 

Changing Your Doctor:
 

If you are outside our area or if you are not satisfied with the service we provide you are free to change doctors. Please contact Havering PCT on 01708-465000 on the procedure.

Doctor is also entitled to remove any patient from the list if she feels the Doctor-Patient relationship has broken down.

“Help Us to Help You”. If you frequently DNA an appointment or cancel an appointment at the last minute (more than three times) doctor may remove you from the list. You will be informed before removal takes place. Those appointments could have been given to more needy people.

 

 

Complaints Procedure:
 
We always try to provide the best service, but at times mistakes do happen. If you have any problems with the service you are getting we would appreciate it if you come and discuss the matter, either with a practice amanager or with Dr Sudha. We are committed to provide a personal service. If you wish to make a complaint, please write to our practice manager who will contact you and, in accordance with NHS guidelines, will address the issues.

 

Video Consultations:
 
As this is a training practice we sometimes video the consultations. This is to improve the consultation technique of GP Registrars. We do take your prior permission. If you do not want to be videoed please inform the receptionist.

 

Confidentiality:
 
We respect your confidentiality which is maintained at all times. All the staff are bound by a confidentiality clause in their employment.

 

Private Referrals:
 
Please allow 5-7 days for letters to be typed. It is your responsibility to pick up the letter and to make the appointment.

 

NHS Referrals:
 
We will post them through the courier to local hospitals. When the hospital contacts you please re-confirm with the appointments office that you still require a hospital consultation. Otherwise the hospital will remove your name from the waiting list.

 

Computerised Records:
 
Ours is a totally paperless surgery. All results and hospital letters are scanned into patient notes.
